1. Police Clearance (Latest/Not expired)

2. Barangay Clearance (Latest/Not expired)

3. Community Tax Certificate (Latest/Active year)

4. Birth Certificate (Local or NSO)

5. Official Receipt (Mayor’s Clearance Fee – P50.00) with one (1) Documentary Stamp

6. One (1) piece 2 x 2 Picture (with white background)
